DMCA Notice of Alleged Copyright Infringement

To be valid under the DMCA, your notice must be in writing and must include the following elements, as required by 17 U.S.C. § 512(c)(3):

6. Accuracy and Authority Statement
   A statement that the information in your notice is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner of the exclusive right that is allegedly being infringed.
